The MediPrepare Patient Context is the collection of both medical and contextual data obtained from the patient about the patient. It is obtained through the use of a smart phone, tablet or any computer. The MediPrepare questionnaire presents only relevant questions and creates additional value through combination of questions and answers. This saves time and enables instant use of information like risk profile, medication, alarm signals or social context. The professional pattern through which it is presented enables each caregiver to find in seconds specific answers or the special context of a patient. Thus he quickly gets to the relevant issues.

Patients are people, and people are very diverse. Patient context encompasses things that make us who we are, including gender, age, upbringing, and living situation. A claustrophobic patient may not want to be treated in small rooms, or a Jehovah’s witness may refuse blood transfusion due to religious beliefs. To ensure that your doctor treats you and your context, he or she needs to understand factors in your life.

The electronic medical record contains only the information about a treatment the patient had at that specific institution. Currently this is often limited to the last 10 years. Moreover the MediPrepare Patient Context adds the non-medical information. If doctors need information from other institutions is required, time and effort are required. The MediPrepare Patient Context is different. It is ready for a doctor’s visit – the doctor does not have to organize the data anymore. MediPrepare summarizes all the relevant data so the doctor can see it within 15 seconds.

The idea of patient portals is to connect the patient with one health care provider. A patient portal facilitates for instance repeat prescriptions or the scheduling of a doctors appointment. Or it enables the communicaiton of the care provider to the patient.

Mediprepare is different. It is a collection of data from the patient himself that covers his lifetime and all treatments at all institutions he visited. Through MediPrepare the pieces of information are integrated in a pattern that suits the doctor. It gives a structured overview of the Patient Context and is meant to be shared with any doctor the patient wants to. The patient is the primary source of information.

MediPrepare Questionnaires

They have been developed with an holistic perspective of the patient. However, they are flexible in outcome presentation based on the principle ‘just in time’ and ‘just enough’. The questions have been developed based on 92 health determinants. The caregiver does not have to process the answers for routine checks. The software has done that already. This saves time in turning pages and analysis of answers.

MediPrepare questionnaires are fit for purpose. At special moments in the patient journey a different set of questions is relevant. Both the caregiver and the software assure the right questions are presented when relevant. Subsets of questions find their origin in specific specialisms or specific health conditions. These are included in separate Sections. MediPrepare includes only by the discipline validated and certified general questions. These are a perfect preparation for the specialist. The added value arises from the combination of generic and specific anamneses.

Yes. If patients are not digital savvy or do not have a lower level of health literacy completion of the questionnaire can be done with support by another person. The questionnaire is also suitable for non-Dutch speaking persons due to its multi-lingual design.

Especially patients who are searching online and consider what to do when having a health question,  others that seek someone for advice on what to do next their complaint does not disappear or when Dutch is not their first language.

When invited by a caregiver the questionnaire is free for you. If you want to complete the questionnaire on your own account it will cost € 15,- for the Patient Context and € 7,50 for a translation of the Patient Context in another language. For health care providers the cost for implementation varies due to the implementation process followed.

The patient is well prepared, and also the doctor can be more pro-active. It is a two way act and improves the consult experience. More time for the patient, and a more pleasant experience because all necessary information has been made available already by the patient himself. For non-Dutch speaking persons it is a two-sided sword. The patient can complete th questionnaire in his own language and present it to the doctor in any other language, exactly how he needs it during the diagnosis.

Employers and employment agencies for labour migrants or internationals benefit much from MediPrepare Onboarding and Questionnaire services. These ensure the caregiver receives a sound medical dossier – absent without it – and reduce the language barrier when a consult is sought considerably. Private clinics (generic and specialized anamnesis), organizations that have to deal with triage and medical intake at the phone and mental health clinics benefit also from the Mediprepare 360˚ C Patient Context. Also patients in self-management (personalization of the problem analysis, risk profile) and hospitals (specialists, pre-operative screening) benefit from the questionnaires.

The questionnaires can be implemented without any change in each process where a medical intake is needed or a view of the patient’s context is needed. The questionnaires are aligned with international medical standards and Zib’s, and integration with local information systems has been prepared for.

MediPrepare provides the Patient Context in a pattern that assures he can get insight in the patient’s condition within 15 seconds, what will take him otherwise 30 minutes. Moreover, the information has been arranged in a pattern that follows the diagnostic process during a consultation. This is especially true for non-Dutch speaking persons like labor migrants, internationals and international students. The patient contributes by completing the questionnaire beforehand. Thus the risk for misdiagnosis is limited and creates more time for interaction with the patient.

The Patient Context presents the answers as received from the patient. It is comparable to the face-to-face situation when the doctor receives the response on his question. The difference with face-to-face is that the patient can pay more attention to formulate the answer or think about the it before answering, or he may even check data in his current file or from the medication he is currently taking.

Implementation can be done overnight. If you add a link on your website at ‘make an appointment’ and request the patient to complete the questionnaire and take it with him the first visit it works immediately. Integrating it in the work processes can be done in different ways. MediPrepare has experts to advice you on exchange of medical information and the change of processes.

I am a patient

The Patient Context is valuable at different points in the patient journey. At each contact with your caregiver it is important that the whole Patient Context is available. Every patient is different. Moreover the contact with your caregiver will immediately be more pleasant and lead to an optimal outcome. If more caregivers are involved in your care process it is an easy way for the caregivers to share your Patient Context in which besides all relevant data also your specific needs and experiences as a patient are included.

On average it takes ca 23 minutes. Less than 20 minutes when you are a healthy person, and >30 minutes when having special conditions, medication or risks.

You may complete the questionnaire as you wish. After pausing you can always proceed where you left the questionnaire.

You can save the Patient Context on your own device, or in a Persoons Gebonden Omgeving (PGO). You can share as you wish – on paper or digitally. It is also possible to send it by email to your caregiver who can attach or include it in your medical record. MediPrepare has taken measures that this can be executed in a secure and safe way.

The Patient Context can be shared from his own device via NEN certified email. He may also share the Patient Context by giving the caregiver access to his Persoons Gebonden Omgeving (PGO). Or he may upload it from the PGO to the caregiver environment.

Non-native speaking patients benefit immediately since the MediPrepare questionnaire is multi-lingual and resolves the language barrier. Foremost patients with more than one condition benefit or those patients that are treated at several health care institutions. Also patients who want to share non-medical context or needs with the caregiver.

That is not needed. If you would like to use the Patient Context when travelling or when seeing a caregiver for the first time you may always complete the questionnaire on your own account.
